Welcome to 80-20 Options NZ Ltd’s on-line Risk Management benchmarking tool, the ISO 31000 Rx. You have chosen to complete the ISO 31000 Rx, an on-line diagnostic assessment tool powered by Q100 software developed and proven by QLBS.com.

Risk Management has been described as 'all the things you need to do to make the future sufficiently certain'. A ‘best practice’ risk management framework, principles and processes are described in the ISO 31000, the International Risk Management Standard: 2009.

The ISO 31000 Rx:

  • provides a benchmark against which organisations are able to assess their performance in these areas and offers the opportunity to focus on and structure meaningful organisational risk management improvement projects;

  • focuses on your organisation’s existing risk management culture, management and staff attitudes towards risk and their risk management practices and processes; and

  • provides a structured assessment of the extent of integration and the consistency of risk management practices across the organisation and across different types of risk.

Survey results will be used to identify existing processes and systems that can be improved on and any constraints limiting the introduction or implementation of systematic risk management.

The results of the Survey will assist your organisation to identify and prioritise areas that can be improved on further. These will be used to develop an easy-to-implement action plan for better risk management in the organisation.


General Instructions for completing this Survey Questionnaire

This Survey Questionnaire has 2 sections containing statements about Risk Management as follows:

  • Section 1: your organisation’s risk management framework and adoption of risk management principles; and

  • Section 2: your organisation’s risk management processes / practices

Please read each statement carefully, then record your response as either YES, NO or NOT APPLICABLE, based on your first thoughts / response to each statement. Your first response often best describes the way it is or the way it is for you (your perception) and should be recorded as the most appropriate response.

Each YES response scores ‘1’ and both NO responses and n/a responses score ‘0’.

Individual scores out of a possible total score of 131 will be aggregated into Team, Division and Organisation Risk Index and profiles by statement /question and by section. A summary graph(s) will indicate the extent of YES, NO and / or n/a responses. Scores will also be available for Risk Framework/ Risk Principles and Risk Processes and Practices.


There are no right or wrong answers, but an honest assessment will provide useful information on which to base future action plans related to risk in your organisation. At the end of each section, space has been provided in the survey questionnaire response format for you to add any additional responses related to the questions in that section.

For sake of clarity, “organisation” refers to the broader corporate organisation and “work team” refers to the group of which you are a permanent member and work with closely.

Confidentiality
Your responses will remain confidential. Responses will be processed into an overall profile for the organisation, group or team. Although the overall results may be used in future workshops and team discussions, your individual responses will not be divulged and / or discussed, unless you specifically volunteer the sharing this information. The unique identifier codes that were provided to you to log-on and complete the questionnaire on-line will only be used to group data for recording and presenting aggregated responses.
